National provider records
The directory uses individual-provider records from the CMS NPPES NPI Registry, covering chiropractors and pain-medicine taxonomies. MD and DO credentials appear as reported by providers. We retain one record per NPI, a practice location, phone number when available, and source identifiers. An active NPI record is not proof of active state licensure. The database is a partial snapshot, not a complete national census, and is not continuously updated.
Jacksonville-area clinic locations
33 clinic locations were added from official clinic websites, including 21 Injury Care Centers locations and affiliated offices. Google listings and the supplied Mandarin Google Maps listing were used for discovery; clinic websites supply contact details and logos. Each location is listed separately. Individual clinicians may also appear in the NPI records.
Injury Care Centers locations appear first among matching results, followed by the other featured local clinics. Featured placement is directory ordering, not a ranking of medical quality, verification of licensure, or confirmation of a partnership.

We consolidated the duplicate ICC St. Augustine / St. Johns address under ZIP 32092. Oceanway has conflicting suite numbers on the clinic website; call before visiting. Affiliate offices are labeled separately and should be contacted to confirm location and appointments. The University office is an MRI imaging location, not a chiropractic treatment office.

Other states
National NPI records support searches in all 50 states. Individual state rosters beyond Florida, Georgia, and Texas have not yet been imported. The National Board of Chiropractic Examiners state-board directory helps locate the appropriate regulator.

Verify the current license and any restrictions with the relevant board. Confirm the practice location, appointment availability, accident-care services, and billing directly. Public listing does not imply a Wreck Doctors partnership.
